  • Preventive Drug ListFor use under the Basic and Choice Savings medical plans. Medications on the preventive list do not apply to the annual deductible; therefore, you pay the pharmacy co-pay amount or 25 percent (whichever is greater)

  • Mail Order Registration FormSave money on high cost maintenance medications through the mail order program. Simply ask your doctor for a 90-day plus refill prescription and return it along with your registration form

  • Wellness Reimbursement FormIf eligible, Ruan may reimburse 50 percent up to $200 per year per family for fitness facility fees, a weight loss program or smoking cessation course
